Registration Form

I understand my student is registering for an activity that involves participation in group activities. This necessitates working collaboratively with other students and listening respectfully when others are speaking. I also understand my student’s completion of homework assignments is important to the learning and enjoyment of my student and of the entire group.

I understand classes are not recorded. Attendance is important. For live online classes, I also understand my student is expected to have a working webcam and mic system for every class. For in-person classes, I understand I am expected to drop off and pick up my student on time.

I understand a full refund is provided only if class is canceled due to insufficient enrollment. Otherwise, the registration fee is non-refundable unless the class is fully enrolled and another student can be found to fill your student’s spot. Registration is complete and your student's spot is reserved only when payment has been received.

I understand all class materials are the intellectual property of Learning Outside the Box and may not be used by any other individual or entity in any form for any purpose.
